Veterinarians assisting with marketing cattle

 

1.   Veterinarian must meet the Veterinarian Eligiblitly Requirements and
      submit the following documentation to the VIP Coordinator:
      a.  VIP – Veterinarian Information Sheet
      b.  Pre-Conditioning Form including EID numbers for group of feeder calves their client is marketing
     
2.  VIP Coordinator will then mail the producer
     a.  Grant Authorization and Payment Form
     b.  Substitute W-9 (if W-9 hasn't been returned).

Payment will not be issued to the Veterinarian until cattle have been sold and proof has been submitted to VIP Coordinator by the producer.
